To help us provide the best care from the very first visit, all new patients are required to complete our registration before their initial appointment. Once registered, we will email you over the new patient forms for you to fill out. These forms allow us to gather important information about your child’s history, strengths, and needs. You’ll also be asked to enter your insurance information to help us verify eligibility.

For patients who are out of network, we will gladly provide you with a “superbill”. A superbill outlines the services that were provided and paid for, and may be submitted to your insurance company for reimbursement.
